Film ini bercerita tentang Alice yang terjebak di dalam simulasi bawah air milik perusahaan jahat Umbrella. Dia harus melawan pasukan zombie dan makhluk mutan sambil berusaha melarikan diri. Jujur, film ini jelas bukan untuk anak-anak. Rating R-17+ sangat pas. Adegan kekerasannya intens dan berdarah-darah, bahkan untuk remaja 15 tahun ke atas mungkin terlalu berat.
